Make a Referral

Service users may come to our community-based mental healthcare and rehabilitation services on either a long-term or short- term basis. Long-term service users typically stay for 12-18 months before they are considered ready for move-on. Service users may also join us for period of assessment, so that we can prepare a written report and recommendations on future care options. These may include individuals requiring assessment for budget -holder status under the Personalisation Agenda. Referrals must be made in writing by a social worker or representative of the prospective service user's funding agency. Reports of the service user's social and psychiatric history must accompany our referral application form, together with details of any offences, and we require full disclosure at the time of referral.

We work with all types of service users, including those on step down to supported living from prison or secure hospital, arsonists and sex offenders but we are unable to admit paedophiles. Once we receive the required documentation, we carry out a detailed assessment of the service user at their current home, to identify appropriate risk management strategies and determine key areas of clinical interventions and care provision. Our prompt preparation of the assessment minimises the waiting period between referral and funding panel confirmation.

Post-assessment and pre-funding panel, we are in regular contact with the referrer and service user. Prior to admission, all service users are also invited to make informal day visits to the relevant home and to stay overnight to meet staff and fellow residents. They also receive a pre-admission guide to our services.We welcome visits to our  mental health care homes and supported living facilities by social workers and other referrers.
